Prior to your first appointment we will upload forms into your patient portal for you to complete in advance. If you would prefer a paper copy, we are happy to forward these to you or just arrive to your appointment about 10 minutes ahead of time.

If your condition requires further medical evaluation or intervention, your therapist will refer you to the appropriate specialist. They will also collaborate with your doctor to ensure comprehensive and coordinated care.

For the vast majority of patients, a referral is not needed! Give us a call and we can help you get scheduled without needing to see another provider first.

We are in-network with most of the major insurance carriers in Central Wisconsin. Please give us a call or send us a message and we can help answer any insurance related questions you may have!

Some insurance companies do not provide adequate reimbursement to cover today’s cost of delivering high quality up to date care, or they may feel they have adequate coverage for their patients in this geographic area. We offer a discounted pay up front “Cash” fee where the client and provider are better able to determine the optimum plan of care for the individual without the insurance company’s restrictions. We then provide you with the needed documentation and paperwork to submit your expenses to your insurance company, to seek reimbursement if you so choose. In many cases our clients can receive more care for their dollar than if they were to go through the traditional insurance model. Clients also are better able to dictate how often they would like to attend therapy, as well as direct your care to your desires, not the insurance companies.

Every patient has the right to see any PT or OT they desire! While your doctor or medical provider may suggest certain therapists, it is ultimately your choice on who you want to see. We think you will recognize a difference in the Pinnacle Approach.

No, you have the freedom to choose any physical or occupational therapist you prefer. While your provider may suggest a therapist within their system, you are not required to go there. At Pinnacle Performance Therapy, we work closely with your referring provider and ensure open communication to provide you with the best care possible.
